LiveWell BeWell! The Mange Your Chronic Disease (MyCD) Program

 

This article is part of a project of the local Community Health Council (CHIC).  Health Councils were created statewide in conjunction with the New Mexico Department of Health (NMDOH) with the goal of getting local citizens and county governments more involved in the manner in which health might be improved in each of their communities.  The local CHIC is conducting an ongoing awareness campaign addressing the various areas that can potentially affect a person’s health. January highlights NMDOH “Manage Your Chronic Disease Programs (MyCD).”

 

What is your resolution for the year 2016? Do you want to feel better? Be in control? Do the things you want to do? Put life back in your life by attending a FREE six-week MyCD Program workshop!

 

NM’s MyCD Programs are a family of evidence-based self-management programs designed and tested by the Stanford School of Medicine Patient Education Research Center and include the following programs: Chronic Disease Self-Management Program (CDSMP), Diabetes Self-Management Program (DSMP), and the new cancer program, Thriving and Surviving Program. These programs are nationally and internationally recognized.

 

NM’s MyCD Programs are peer led education workshops offered throughout San Juan County in community settings such as senior centers, churches and community centers for 2 ½ hours, once a week for six weeks. These workshops address the many chronic health conditions that not only financially strain our state and local economies but sadly diminish New Mexican’s quality of life. NM’s MyCD Programs are for adults of all ages with chronic health conditions like diabetes, cancer, arthritis, asthma, heart disease, high blood pressure, or any other ongoing or long-term health condition. NM MyCD Programs will help equip participants to take control of their chronic health conditions and live well.

 

NM’s MyCD participants acquire skills and techniques needed to self-manage their chronic conditions, including how to: gain confidence and motivation to better manage their health; make a step-by-step action plan to improve their health; problem solve and make informed decisions; eat healthy and exercise safely; deal with difficult emotions; and communicate more effectively with their doctor, family or caregiver about their health. NM’s MyCD participants report experiencing better health, better health care, and lower health care costs as they practice the skills and techniques learned at the NM’s MyCD Programs workshops.

 

NM’s MyCD Programs are being imbedded in San Juan County through the collaborative efforts of the New Mexico Department of Health (NMDOH), Community Health Council (CHIC), Presbyterian Medical Services, Molina Health Care, Non-Metro Area Agency on Aging, San Juan Safe Communities Initiative, Basin Home Health & Hospice, Inc., Basin Coordinated Healthcare, Inc. and Partners Assisted Living Services. Thru the dedicated collaboration of many, the NM’s MyCD Programs will pro-actively empower residents of San Juan County to take control of their chronic health conditions and put life back into their life in 2016 and beyond, LiveWell BeWell!
