This article is part of a project of the local Community Health Improvement Council (CHIC). CHICs were created statewide in conjunction with the New Mexico Department of Health (NMDOH) with the goal of getting local citizens and county governments more involved in the manner in which health might be improved in each of their communities. The local CHIC is conducting an on-going awareness campaign address all the areas that can potentially affect a person’s health. February has been designated by the CHIC as the month to bring awareness on the Continuum of Care offered in San Juan County.

 

What exactly is A Continuum Of Care?

 

Good Samaritan Society – Four Corners Village was designed with the understanding that our individual healthcare needs change as we age. We call it a continuum of care. Our senior community offers a variety of housing options, including Long Term Care, Skilled, Memory Care, Senior Living with Services, Assisted Living, and End of Life Care. Services range from inpatient therapy: physical, occupational and speech therapies, beauty shop, and eat in dinning room. The Village Transit provides more than 5,000 shopping and physician trips to community members every year. Good Samaritan – Four Corners Village goals are to add more services lines to the continuum of care such as home health, hospice, physical fitness center, and outpatient services. Which other Good Samaritan Facilities have done nationwide. Currently, Good Samaritan is the only facility in San Juan County to offer the Continuum of Care.

 

But Good Samaritan Society – Four Corners Village is about more than the buildings and amenities. Its difference rests on the philosophy that everyone deserves to feel loved and valued and to be treated with dignity.

 

While caring for others’ physical needs is important, it is equally imperative to meet their emotional and spiritual needs. We provide those we serve with something that not every organization can: the opportunity to experience spiritual and emotional growth. We have an onsite Chaplin, with daily devotionals, and chapel services. To enhance our emotional needs, we offer an array of activities for all of our residents, senior college, and opportunities to be part of the community they love.

 

We seek to share that love in word and deed with others, so that they can feel that they truly have found a place that they can call home.
